Forms & Publications: Resources for Departments
Departments can access forms and publications for their employees in two ways.
- Most forms and publications are available on the systemwide HR/Benefits website: At Your Service located at: http://atyourservice.ucop.edu. Once at the site, click on "Forms and Publications", located on the blue bar running across the top of the page.
- In addition, hard copies of most forms and publications can be ordered from the KP Corporation. A listing of the forms and publications that are available is available on the KP website located at: http://apps.kpcorp.com/ucbenefits/default.htm. In order to access the KP website, you will need a user ID and password. Each department is assigned a user ID and password. If you need your department's information, please contact the benefits unit.
