[1]PIE: Period of Initial Eligibility. Lasts for 31 days beginning on the date of the eligible event (e.g., date of hire, newborn dependent's date of birth).

[2]OE: Open Enrollment. Occurs once a year, usually around the end of October and part of November. Plan changes and rates for the following calendar year are available. Employees who want Flexible Spending Accounts for the following year must enroll or re-enroll. Employees can make changes to enrollments or add eligible family members to the Medical, Dental, and Vision plans. The Legal plan is available for enrollment occasionally.

[3]SOH: Statement of Health. Requires that employee completes a Statement of Health form and that the insurance carrier approves enrollment. This process can take several months. SOH forms can be obtained from HR Benefits.

[4]90-day: employee can enroll in coverage with a 90-day waiting period. Submit UPAY 850 Enrollment form to HR Benefits. Enrollment is on the 91st day after HR Benefits receives the UPAY 850 form.

[5]Auto: If an eligible employee does not enroll in Medical, Dental or Vision coverage, the employee will default into single party Core, Delta and VSP, as appropriate.
