II. BENEFITS PROCEDURES

EMPLOYEES RETURNING FROM A LEAVE OF ABSENCE

  1. Employees must reenroll in benefits within 31 days of their return from a leave of absence using forms (UPAY 850 and/or UPAY 717). Contact Payroll Benefits at 642-1336, ext. 6, to find out which benefits will resume automatically and which benefits the employee needs to reenroll in. This will vary depending on which benefits the employee elected to keep through direct pay during their leave.

    If an employee is on leave for less than 120 days, the employee can only reenroll in the benefits he or she had prior to the leave. If the length of the leave is 120 days or longer, the employee is allowed to reenroll as a newly eligible employee and is not limited to pre-leave benefits enrollments.

  2. UCRP Service Credit buyback information: Give the employee the UCRP Buyback Booklet (UCRP service credit) and the Buyback Factsheet "Buyback: Is it the Best Place for your Money?" Note: There is only a three-year window to elect a buyback. Employees with questions or needing assistance should be referred to an OHR Benefits Unit Retirement and Savings Analyst at 642-7053.
