About the SRA Evaluation form
In
Adobe Acrobat (.PDF) Format
If you have the free version of Acrobat Reader, you can complete
the .PDF version of the form on line, print it out, and
send it to Compensation via campus mail (Human Resources,
MC 3540).
If
you have the full retail version of Adobe Acrobat (less than $100
from The Scholar's Workstation) you can complete the form on line,
save it, attach it to an
e-mail, and send it to Compensation at compdesk@uclink.berkeley.edu.
Mac Users: You must have "Adobe Acrobat Reader" to download and use the PDF version of this form. Instructions for downloading and installing Acrobat reader are available at http://hrweb.berkeley.edu/forms/formlist.htm. Be sure to use a .pdf file extension in the filename.
PDF tips:
- As you scroll over the form, your cursor appears as a hand icon.
- Move the hand icon to the any field and left-click to enable the cursor.
- Use the tab buttons to move from line to line.
Note: If you routinely use keyboard shortcuts when editing documents, we suggest that you use your usual word processor for large blocks of text and then cut-and-paste them into the .PDF form. Some standard keyboard shortcuts perform different functions in interactive Acrobat forms and it can be frustrating for those who use keyboard shortcuts.
In
Microsoft Word Format:
The
Word version can be completed on line, saved, attached to an e-mail,
and sent to Human Resources at
compdesk@uclink.berkeley.edu
or it can be completed on line, printed out, and sent to via campus
mail to Human Resources MC 3540.
You may wish to save it wherever you normally save documents
on your computer (e.g., Worddocs) with an explanatory name and
.doc extension.
For Mac Users: You must have a recent version of Word (MS Word 98 or 2000) to save and send the electronic form so that we can access it.
To complete the form:
- Move your cursor to the grey space in any field. The grey space supports an unlimited amount of text, although it looks small. Keep typing and it will expand.
- Use the tab buttons to move from line to line. Do not use the Enter key to add lines.
- When you have a choice of boxes, such as Exempt or Non-Exempt, enter an X in the box that reflects the correct choice by clicking inside the box. Please check only one box where appropriate.
