The Final Assessment Document

Record the results of your discussion on the performance assessment form. Ask the employee to sign and date the form, and explain that this signature acknowledges discussion of the contents, not necessarily agreement with them. Route the document to your manager for final signatures. Give a copy of the signed assessment to the employee, and place a copy in the personnel file.
