CHANGE OF STATUS LETTER FOR UNEMPLOYMENT PURPOSES

RE: Unemployment Insurance

As a result of the change in your employment status effective __________, you have the right to file for unemployment insurance benefits with the California Employment Development Department (EDD). The enclosed EDD Booklet, "California's Programs for the Unemployed" describes California’s programs for the unemployed. Please note that you can now file a claim for benefits online at http://www.edd.ca.gov. Claims can also be filed by telephone. Page 2 of the EDD Booklet, "California's Programs for the Unemployed" has a list of numbers to call according to your choice of language.

Your eligibility for benefits will be determined by the EDD.

You may bring this letter with you if you file for unemployment benefits in person. The change of status information below may expedite your application for benefits.
